Form for Requesting a Facilities Remodel (a.k.a., Requesting to Move Your Office)
If you are requesting to move locations, this form must be completed. (See Attachments)
This form must be completed, approved/signed by your Dean/Director, evaluated by M&O and IT/IMS for labor and/or materials costs, and then forwarded to the Facilities Committee for action/approval.
The Facilities Committee will then forward it to the V.P., Admin. Services for funding of the project.
